Submitting a Claim

Manulife manages our medical, prescription drug, dental, personal wellness, and health care spending accounts. To submit a claim online, you can use the Manulife app. To submit a claim through the mail, visit the Manulife website to get the form you need. To have your provider – medical, pharmacist, dental office, optical location or paramedical – submit a claim on your behalf provide them with the appropriate policy number.

Using Manulife to Submit a Claim

To get started using Manulife App:

You can start submitting claims after you complete 3 easy steps:

Go to https://www.manulife.ca/personal/sign-in.html Click Sign In under “My Group Benefits” to either set up your Manulife ID, or login. If you already have a Manulife ID from your Group Pension with Manulife, skip to step 2.
After setting up your Manulife ID, you’ll need to activate your benefits plan by registering with the following information:
• Contract number: 146509
• Certificate number: Employee Number
• Name and date of birth
At the end of the setup process, click Go to access the site, then navigate to My account tab to review and/or update personal details.

Tip: To get your money back faster through direct deposit, enter your
banking information.
